Dilated Cardiomyopathy Mimicking Arrhythmogenic Right Ventricular Cardiomyopathy in a 35-Year-Old Woman

چکیده

While dilated cardiomyopathy (DCMP) typically involves the left ventricle (LV), it can also impact right (RV). Arrhythmogenic ventricular is a heritable myocardial disorder that mainly affects RV. A 35-year-old woman presented to emergency department complaining of chest discomfort and palpitations. The patient had ingested 100 g alcohol daily for more than 5 years. Transthoracic echocardiography showed dilation four cardiac chambers with global systolic dysfunction prominent recesses both ventricles. Non-sustained tachycardia was observed on 24-hour Holter monitoring. On magnetic resonance imaging (MRI), ratio between noncompacted compacted layers did not meet criteria non-compaction, delayed gadolinium enhancement only in inferior LV myocardium. Endomyocardial biopsy revealed no fibrosis or fatty changes. Consequently, diagnosis alcohol-induced DCMP reached. Various diagnostic modalities, including MRI biopsy, contribute accurate diagnoses.

Arrhythmogenic right ventricular cardiomyopathy in a 20-year-old woman with systemic sclerosis.

A 20-year-old Japanese woman with systemic sclerosis was evaluated for the progressive aggravation of chest pain, palpitations and dyspnea. Cardiac magnetic resonance imaging revealed diffuse wall thinning, segmental dyskinesis and late gadolinium enhancement of the right ventricular (RV) myocardium. Cardiac catheterization demonstrated no pulmonary hypertension. 24-hour electrocardiography mon...

Arrhythmogenic right ventricular dysplasia/ cardiomyopathy versus dilated right ventricular cardiomyopathy: a problematic autopsy diagnosis?

Arrhythmogenic right ventricular dysplasia-cardiomyopathy (ARVD-C) is a rare heart condition with characteristic thinning and fibroadipose tissue replacement of the myocardium of the right ventricular wall. We have seen 20 autopsy cases with morphologic features fitting the criteria of ARVD-C in our hospital in the past 5 years. The clinical characteristics of these patients were not those desc...

Arrhythmogenic Right Ventricular Cardiomyopathy

Arrythmogenic right ventricular (RV) cardiomyopathy (ARVC) is a cardiomyopathy characterized pathologically by fibrofatty replacement primarily of the RV and clinically by life-threatening ventricular arrhythmias in apparently healthy young people. The prevalence of the disease has been estimated at 1 in 5,000 individuals, although this estimate will likely increase as awareness of the conditio...

Arrhythmogenic right ventricular cardiomyopathy.

Arrhythmogenic right ventricular cardiomyopathy (ARVC) has evolved from postmortem pathology at to a diagnosable clinical condition, and holds promise for definitive genetic diagnosis. Its prevalence is between 1/1,000 and 1/5,000, with 10% of deaths occurring before age 19 and 50% before age 35. When analyzed against age-specific norms, the electrocardiography (ECG) and signal-averaged ECG (SA...

Primary prophylaxis of sudden death in hypertrophic cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy, and dilated cardiomyopathy.

We present an evidence-based overview of primary prevention of sudden cardiac death. Several recent studies have provided important data regarding pharmacologic and device-based therapy for patients with conditions that confer high risk for sudden death. A rational approach to these therapies, with emphasis on implanted cardiovertor defibrillators, is discussed.
